Root rot problems

Had root rot on my first 2 grow using roots organic soil with added perlite what do you guys do to control this?

... sounds like the pot has poor drainage, or got pugged up,.. [HASHTAG]#1[/HASHTAG] cause of root rot is overly wet soil,....

Only watered when needed but I think my pH was getting fucked up at one point can that cause it
